What will you be doing in this role?

You will deliver a full range of social work services in

collaboration with other CSMC staff to meet the psycho-social assessments,

crisis intervention, Care Coordination, transitions management and discharge

planning needs of patients and families.

Masters Degree in Social Work required.

Clinical experience in an acute care

hospital required
