For over many years, Cedars-Sinai has been on the cutting edge of neurosciences research and advanced clinical care of complex neurological conditions. The Jona Goldrich Center for Alzheimer's & Memory Disorders is a newly-established clinical and research hub for Alzheimer's disease and related dementias (ADRD) that brings together the skills of a multidisciplinary team of clinicians and researchers throughout the Cedars-Sinai Health System. The Center's Memory & Aging Program will deliver comprehensive diagnosis, treatment and care management of patients with cognitive impairments, person-centered support for their families and access to the latest ADRD research studies.
Summary of Essential Duties:
In this newly created Nurse Practitioner role, you will deliver direct care and coordinate the care of Cedars-Sinai Health System patients with Alzheimer’s disease and other forms of dementia
Participate in interdisciplinary neurobehavioral clinics, provide education and support and make referrals to other services/specialties as appropriate
Develop and implement individualized dementia care plans based on needs assessments
Communicate care plans to physicians and other health practitioners to ensure continuity of care
Conduct in-person and/or virtual sessions at which patient and family members' specific questions about problems, resources, and implementing care plans are answered
Perform telehealth/telephone follow-up to monitor implementation of dementia treatment and care plans
Facilitate appointments with consultants when the treatment plan needs to be reassessed (e.g., new neurologic, medical or behavioral symptoms)
Teach dementia education, management and coping skills to caregivers through individual counseling including information on: legal and financial planning with referral to community services, behavioral techniques to avoid/manage behavioral problems, and coping strategies for caregivers
Refer, when appropriate, to community-based organizations for services and resources
Master'sDegree, Nursing (MSN) required
Graduateof an accredited Nurse Practitioner program required
Current,unrestricted California RN and NP License required
BLSfrom the American Heart Association or American Red Cross required
DEAlicense required or eligible to obtain
3years of Acute Care nursing experience preferred
Experienceas an Advance Practice Nurse preferred
Experienceworking with Dementia and/or Alzheimer's patients preferred
