Essential Duties:

As a Clinical

Program Coordinator, Transplant (CPC) you will be the clinical expert who serves

as the navigator and liaison between the transplant programs, the patients and

the referring physicians; the CPC supports the vision of the medical and

surgical program directors in planning continuing medical and nursing education

opportunities in the greater Los Angeles area.

  • Createsnetworking opportunities for faculty and referring physicians in thecommunity

  • Createsphysician speaker series schedule and conduct presentations, maintainrelationships with collaborative partners and cultivate new ones

  • Follow-upwith physicians following a transfer or a referral

  • Roundson inpatients to ensure smooth transition process

  • Participatesin the development of future satellite clinics; serves as main point ofcontact between target facilities and Cedars-Sinai

  • Ensuresall Cedars-Sinai logistical needs are in place prior to new clinic startdate

  • Hasa presence at all Satellite Clinics and provide patient education at theselocations; provide nursing care and complete initial patient intake anddocument in EMR

  • Organizesand coordinates CME courses and community lectures

  • Ensureseducation materials are current

  • Mainresource at the satellite clinics between referring MD, clinic staff andCS admin and clinical teams

  • Maintainsdata tracking of referrals

  • Providescross-coverage for other CPCs during vacation or leave of absence


  • BSN required

  • MSN preferred


  • Current, unrestricted California RN License required

  • Specialty or Transplant Certification required or willing to obtain within one year from start date

  • BLS from the American Heart Association required


  • A minimum of 3 years of nursing experience in transplant or acute care required

  • Demonstrated knowledge of case management required

  • Experience in Data Management required

  • Experience in coordinating large-scale community-based projects preferred
